REGISTRATION - 2012
To become a member of the league you must complete and sign a League Registration Form, pay your dues and purchase a membership in
Alberta Senior Citizens Sport and Recreation Association (ASCSRA). You canot play in our league without the accident
insurance coverage offered by ASCSRA. Make sure you sign page 2 of the ASCSRA membership application.
For a description of the three divisions referenced on the Calgary Senior Men's Application Form go to the About Us page on this website.
The forms required to register are listed below, If for some reason you cannot download and/or print the required forms contact us by email at
webmaster@grandslamslopitch.com. Please give us your name and phone number.
PAYMENT: Two (2) Cheques are required:
** A cheque made payable to Calgary Senior Mens's Slopitch. The amount you pay will depend on when you register.
Refer to the Registration Form for details.
If you do not register by April 23, 2012 your name will not go in the draft.
** A cheque for $30 for ASCSRA. If you live in Calgary, make the cheque payable to CRCA,
if you live outside of Calgary, make the cheque payable to BCSS.
Mail the completed forms and your two cheques to:
